You treat in the patient’s home rather than in a clinic, which means a caseload measured in visits rather than in back-to-back appointment slots, and evaluations that happen where the patient actually has to function — their own stairs, their own bathroom, their own kitchen.
Every position is a 1099 contract. You decide how many visits you want, which days you work, and how far you are willing to drive. Some of our PTs take a single patient alongside a hospital job; others build a full week. Both are normal here, and you can move between them without renegotiating anything.
What the work involves
- Evaluations and re-certifications in the home, including gait, balance, transfer, and fall-risk assessment against the patient’s real environment.
- Plan-of-care development and supervision of the PTAs carrying out treatment between your visits.
- Post-surgical and post-hospitalisation recovery — joint replacement, cardiac events, and deconditioning after a long admission.
- Neurological caseloads including stroke, Parkinson’s, and progressive conditions where the goal is maintaining function rather than restoring it.
- Documentation in our own EMR, which has AI assistance built in specifically so charting does not follow you home.

What you need
- Current Texas physical therapist licence in good standing.
- CPR certification and current professional liability coverage.
- Reliable transportation and current auto insurance — this is a driving role.
- Credentialing before the first visit: licence verification, background screening, exclusion-list checks, and health requirements including TB screening.
