Home health occupational therapist jobs across Texas
OT contracts where the treatment environment is the patient’s actual home, not a simulated one.
Occupational therapy in the home removes the usual gap between the clinic and real life. You assess someone getting out of their own bed, using their own shower, and managing their own kitchen — so the equipment recommendations and compensatory strategies you write are the ones that will actually hold once you leave.
Universal Rehab places OTs with home health, hospice, and HCS and waiver provider agencies across Texas on 1099 contracts. You set the caseload, the hours, and the coverage area, and the back office handles scheduling, authorisations, and billing.
What the work involves
- ADL and IADL evaluation in the patient’s own environment, including bathing, dressing, toileting, and meal preparation.
- Home safety and fall-risk assessment, with adaptive equipment and durable medical equipment recommendations.
- Upper-extremity rehabilitation and functional strengthening after stroke, fracture, or prolonged hospitalisation.
- Cognitive rehabilitation and compensatory strategy training for patients with memory or executive-function change.
- Caregiver training, and supervision of the OTAs delivering treatment between your visits.

Pay, schedule, and support
- You set the caseload. As a 1099 clinician you choose your visits, hours, and coverage area — from one patient at a time to a full load.
- Per-visit pay, paid weekly. Competitive per-visit rates at or above market, with weekly direct deposit and transparent payroll.
- Charting that ends with your day. Our own EMR has AI assistance built in so documentation does not follow you home.

- Current Texas occupational therapist licence in good standing.
- NBCOT certification, CPR certification, and current professional liability coverage.

Questions occupational therapists ask
Is home health OT different from clinic work?
Substantially. You are assessing function in the environment where it has to happen, so equipment and strategy recommendations can be tested on the spot rather than approximated in a treatment gym.
Can I choose how many visits I take?
Yes. Every position is a 1099 contract, so the caseload is yours to set — from a single patient alongside another job to a full week.
What kind of patients would I see?
The caseload stays varied, from paediatrics to geriatrics, because referrals come from home health, hospice, and HCS and waiver programmes rather than a single source.
