Physical Therapist — Houston, TX
About the Role
As a Houston-area PT with Universal Rehab, you evaluate and treat patients in their homes, build individualized plans of care, and document through our proprietary EMR. You work one-on-one — no rotating staff, no productivity quotas. Strong interdisciplinary support from our OTs, SLPs, nurses, social workers, and dietitians rounds out every plan of care.
Key Responsibilities
- Perform initial evaluations and ongoing reassessments in patients’ homes across the Greater Houston area
- Design and deliver plans of care for post-surgical, stroke, Parkinson’s, cardiac, and orthopedic patients
- Coordinate with OT, SLP, nursing, and social work through our in-house interdisciplinary team
- Train home caregivers on safe transfers, gait practice, and exercise progression
- Document visits in our proprietary EMR on the same day as the visit
- Participate in case conferences when patients are on complex plans of care
- Cover patients across Memorial, Katy, Sugar Land, Pearland, The Woodlands, and Clear Lake as caseload allows
Requirements
- Active Texas Physical Therapist license (or eligibility via compact license)
- Current BLS/CPR certification
- Reliable transportation and valid Texas driver’s license with clean MVR
- Minimum 1 year clinical experience (home health or acute care preferred)
- Bilingual Spanish a plus given Houston’s large bilingual patient population
- Comfortable using web-based EMR and basic mobile technology
