About the Role

As a Longview-area PT, you run one-on-one home-health visits, build plans of care, and document in our proprietary EMR. East Texas home-health offers a stable patient base, real continuity of care, and an established healthcare community shared with the Tyler metro to the west.

Key Responsibilities

  • Evaluate and treat patients in their homes across Gregg, Harrison, and Upshur counties
  • Design plans of care for post-surgical, stroke, cardiac, and chronic-pain patients
  • Coordinate with our in-house OTs, SLPs, RDs, RNs, and LCSWs on shared patients
  • Deliver gait training, balance retraining, and functional mobility work
  • Educate caregivers on transfers and home exercise programs
  • Document visits in our proprietary EMR on day of service
  • Cover Longview and surrounding suburbs including Kilgore, White Oak, Hallsville

Requirements

  • Active Texas Physical Therapist license
  • Current BLS/CPR certification
  • Reliable transportation and valid Texas driver’s license
  • Minimum 1 year clinical experience (home health preferred)
  • Experience with neuro rehab, orthopedics, or cardiac rehab valued
  • Comfortable with mobile EMR
