As a Beaumont-area PT, you run one-on-one home-health visits across Jefferson County and the Golden Triangle. Beaumont's petrochemical economy shapes a meaningful share of the patient base, and the metro's Gulf Coast hospital systems anchor a stable home-health referral environment.
Key Responsibilities
- Evaluate and treat patients in their homes across Jefferson County and the Golden Triangle
- Design plans of care for post-surgical, stroke, cardiac, and chronic-pain patients
- Coordinate with our in-house OTs, SLPs, RDs, RNs, and LCSWs on shared patients
- Deliver gait training, balance retraining, and functional mobility work
- Educate caregivers on transfers and home exercise programs
- Document visits in our proprietary EMR on day of service
- Cover Beaumont and surrounding Golden Triangle cities — Port Arthur, Orange, Vidor, Lumberton
Requirements
- Active Texas Physical Therapist license
- Current BLS/CPR certification
- Reliable transportation and valid Texas driver’s license
- Minimum 1 year clinical experience (home health preferred)
- Experience with neuro rehab, orthopedics, or cardiac rehab valued
- Bilingual Spanish appreciated
