What the work involves
- Psychosocial assessment of patients and families, including risk, coping, and support-system evaluation.
- Counselling and psychotherapy within the plan of care, including grief and adjustment work in hospice cases.
- Resource coordination — benefits, community services, equipment, and long-term care planning.
- Discharge and transition planning alongside the nursing and therapy disciplines on the case.
