About the Role

Our Houston RNs run a full skilled-nursing scope — assessments, medication reconciliation, wound vac management, infusion nursing, tube-feeding management, and coordination with our broader rehab team. Strong supervision structure, real clinical autonomy, and weekly direct-deposit payroll.

Key Responsibilities

  • Perform OASIS evaluations and ongoing assessments in patients’ homes
  • Deliver wound care, including negative-pressure wound therapy and complex dressings
  • Manage IV therapy, infusions, and central line maintenance
  • Coordinate medication management and reconciliation with primary care providers
  • Support tracheostomy, ventilator, and tube-feeding patients in coordination with RTs and RDs
  • Teach patients and family caregivers on disease self-management and medication adherence
  • Document in our proprietary EMR compliant with CMS home-health regulations

Requirements

  • Active Texas RN license
  • Current BLS/CPR certification (ACLS a plus)
  • Minimum 2 years clinical experience (acute or home health preferred)
  • Reliable transportation and valid Texas driver’s license
  • Home-health experience and OASIS familiarity strongly valued
  • Bilingual Spanish appreciated given Houston patient demographics
