About the Role
Our Nacogdoches-area RNs run the full skilled-nursing scope.
Key Responsibilities
- Perform OASIS evaluations and ongoing assessments
- Deliver wound care including negative-pressure wound therapy
- Manage IV therapy and central line maintenance
- Coordinate medication management with primary care
- Support tracheostomy, ventilator, and tube-feeding patients
- Teach patients and caregivers on disease self-management
- Document in our proprietary EMR per CMS standards
Requirements
- Active Texas RN license
- Current BLS/CPR certification (ACLS a plus)
- Minimum 2 years clinical experience (acute or home health preferred)
- Reliable transportation and valid Texas driver’s license
- OASIS familiarity strongly valued
- Bilingual Spanish appreciated
