About the Role

Our Home Health Therapists bring rehabilitation services directly to patients in their homes. This role offers flexibility and autonomy while making a profound difference in patients' lives, working with post-surgical patients, seniors, and individuals with chronic conditions.

Key Responsibilities

Provide PT, OT, or SLP services in patients' homes
Conduct home safety assessments
Develop home exercise programs tailored to the environment
Coordinate care with home health aides and nurses
Communicate with physicians regarding patient progress

Requirements

Valid PT, OT, or SLP license
Home health experience preferred
Valid driver's license and reliable transportation
Ability to work independently
Excellent time management skills
