About the Role
Our Home Health Therapists bring rehabilitation services directly to patients in their homes. This role offers flexibility and autonomy while making a profound difference in patients' lives, working with post-surgical patients, seniors, and individuals with chronic conditions.
Key Responsibilities
- Provide PT, OT, or SLP services in patients' homes
- Conduct home safety assessments
- Develop home exercise programs tailored to the environment
- Coordinate care with home health aides and nurses
- Communicate with physicians regarding patient progress
Requirements
- Valid PT, OT, or SLP license
- Home health experience preferred
- Valid driver's license and reliable transportation
- Ability to work independently
- Excellent time management skills
